Home Medicines Review


What is a Home Medicine Review (HMR)?

A HMR is a review of a consumer’s medication which occurs in their home to assist them with medication management. A HMR aims to assist consumers living at home to maximise the benefits of their medication regimen and prevent medication related problems.

HMR Process

A HMR involves a written referral from a general practitioner to the patients preferred community pharmacy or accredited pharmacist.  An accredited pharmacist then makes an appointment with the consumer who then visits the consumer’s home and reviews their medication regimen. The accredited pharmacist then provides the general practitioner with a report detailing the consumer’s medications. The general practitioner and consumer then agree on a medication management plan.

The objectives of an HMR are to:
  • achieve safe, effective, and appropriate use of medications by detecting and addressing medication-related problem/s that interfere with desired patient outcomes
  • improve the patient’s quality of life and health outcomes using a best practice approach, that involves a collaborative effort between the GP, pharmacist, other relevant health professionals and the patient (and where appropriate, their carer)
  • improve knowledge and understanding about medications
  • facilitate cooperative working relationships between members of the health care team, in the interests of patient health and well-being
