Diabetes Connect
Connecting Professions; Connecting Online
Diabetes Connect is a collaborative initiative between GPQ and the Queensland divisions of general practice with funding from Queensland Health.
Connecting general practitioners, practice nurses and a variety of primary care providers the initiative aims to:

Enhance communication between general practice and other health care providers

Provide best practice clinical education

Assist patient referral pathways with the iHealth Care+ Directory

Clarify the roles played by various health care professional in the management of patients with diabetes

the online and interactive training system with clinical education module;
- dates and locations of interprofessional learning workshops;
- the state-wide web based iHealth Care+ Directory to include local diabetes services.
