Diabetes Connect

Connecting Professions; Connecting Online

Diabetes Connect is a collaborative initiative between GPQ and the Queensland General Practice Network with funding from Queensland Health.

Connecting general practitioners, practice nurses and a variety of primary care providers the initiative aims to:

• Enhance communication between general practice and other health care providers

• Provide best practice clinical education

• Assist patient referral pathways with the iHealth Care+ Directory

• Clarify the roles played by various health care professional in the management of patients with diabetes
